No way to know.
iOS allows you to pick specific photos to share with an app, you can edit that in the settings.
They could have. Full access is full access.
I doubt discord is trying to save all your photos. It’s just that they technically could have if they wanted to but that’s the absolute worst-case scenario.
truth be told, they might have it, but whether anyone has seen it or not or even wants to see it is very low. discord is such a big company and unless you are some very special poi i doubt they have seen it.
if you were really concerned for discord particularly, read this thread on how to prepare a list of messages that you may want deleted and how to threaten discord with the gdpr to get them to mass delete it. simply deleting your account might not be effective enough for your threat model since they leave the messages up, whereas this gets them to delete the messages too.
the same thing should also be on a video by no text to speech from a few months ago in a more condensed/structured fashion

privacy should be taken in steps. try to take too big of a privacy leap and you end up with privacy fatigue, much like how i started, a big leap right into a faceplant. reading up on this website helped a lot.
there is a site that generally documents how to delete/remove your account/info for basically every site in existence, but i unfortunately forgot what the site was called, if someone else could fill it in for me.
edit: the site is https://justdeleteme.xyz/
